Two things have been mixed together. Calling a spade a spade is an old card playing expression referring to the practice of some cheats to hide most of a club card in their hand to pass it off as a spade thereby completing their hand. On a totally separate wave the word spade from the black card, was taken as a term to be applied to black people. People have mistakenly crossed the two uses and have made a racial slur out of an innocent expression. I hear calling a spade a spade ( meaning calling a thing what it is) used quite a lot so it isn't universally understood to be a racist slur.
